Personalized Planning

At Brockville & Area Community Living Association (BACLA), we personalize the care we give to the people we support by using an Individual Support Plan and holding annual meetings with them, referred to as “My Meeting,” to stay up-to-date on their needs as they change over time.

Individual Support Plan (ISP)

These plans help us provide personalized support tailored to the needs of each person we care for. We use them to review what a typical day might look like for the people we support and how that changes throughout the week. We also use them to document any personal, medical, and financial needs and community resources they might need, like event participation and work opportunities, to live their best lives.

ISPs help us assess the level of support a person requires for domestic needs such as cooking, cleaning, laundry, and phone use. They also help us gauge a person’s ability to safely travel outside their home, their level of mobility, and the amount of supervision and assistance they require to participate fully in the community.

My Meeting

“My Meeting” is an annual meeting we hold with the people we support to formally check in and ensure the personalized care we’re providing is up-to-date with their current needs.

During a person’s “My Meeting,” they, the people they’ve invited and the BACLA support staff take the time to consider significant events from the person’s life, from birth to present, so we can all be mindful of their unique experiences and perspective as we support them in working toward living their best life.

We also use a person’s “My Meeting” as an opportunity to celebrate their positive qualities while reinforcing their confidence and self-esteem. We then spend some time discussing the important people in the person’s life, the positive aspects of their life and the areas of their life they want to improve on.

Next, we account for all the great things the person has done and experienced in the past year and discuss what an ideal future looks like for them moving forward before picking some goals to help them work toward that future and planning strategies to help them achieve those goals.

We finish the “My Meeting” by ensuring the people we support have access to all of the policies and documents we abide by. Going forward, we ensure that the staff the person works with have access to the information they need to help the person they’re supporting achieve their goals and follow their plans.
